PTSD and trauma support

The VA Caribbean Healthcare System offers a wide range of health, support, and facility services for veterans in Puerto Rico, Saint Thomas, and Saint Croix. The VA provides specialty evidence-based time-limited treatment for PTSD, which is a common mental health problem that some people develop after experiencing or witnessing a life-threatening or traumatic event. If symptoms last more than a few months, it may be PTSD, but there are effective treatments available.

The VA offers several treatment programs for veterans with PTSD, and all veterans enrolled in VA care can receive PTSD treatment, whether in person or via telehealth. Each VA Medical Center offers PTSD treatment, and most large Community-Based Outpatient Clinics (CBOCs) also offer PTSD care. All VA health care facilities offer same-day help, and you may qualify even without enrolling in VA health care.

Oncology and haematology services

Although I could not find specific information about VA hospitals in Puerto Rico, I did find some information about transgender veteran care, which is outlined below.

The Veterans Health Administration (VHA) has provided transgender transition-related care to veterans since 2011. On June 9, 2011, the Department of Veterans Affairs issued a directive titled "Providing Healthcare for Transgender and Intersex Veterans," which established the provision of hormone therapy, gender-related mental health counselling, and other transition-related services. This directive also mandated that the VA health system provides care "without discrimination and in a manner ... consistent with the Veteran's self-identified gender." However, it does not include coverage of surgical procedures, although the VA does provide pre- and postoperative care for transgender veterans.

Despite the availability of transgender healthcare coverage through the VA, a study identified five key factors impacting the accessibility and quality of care for transgender veterans:

• Long delays in receiving care;
• Needing to travel to receive care;
• Lack of patient knowledge regarding the coverage of transition-related care;
• Insensitivity, harassment, and violence among providers;
• A general lack of knowledge about transgender patients and care among providers.

Recent research has also demonstrated that transgender people face additional barriers to accessing healthcare, including discrimination, limited coverage, and experiences of harassment and violence in medical settings. A national survey of 827 transgender veterans found that more than one in five reported discriminatory treatment by doctors and staff before the 2011 VHA directive.

The VHA has taken steps to address these challenges by establishing protections against discrimination and expanding the level of care. However, recent policy changes under the Trump administration, such as the ban on trans people enlisting in the military and the rescinding of directive 1341, have created uncertainty and fear among transgender veterans seeking gender-affirming care. These rollbacks have put the availability of treatments like hormone therapy in limbo and have had a negative impact on the mental health of transgender veterans.
