The hospital was established in 1880 as the "Asylum for Colored Insane", serving as the only mental health institution for the African-American population in North Carolina. Over its first 85 years of operation, the hospital served 91,045 patients. The name was changed to Cherry Hospital in 1959 to honour Governor R. Gregg Cherry, who focused his administration on expanding mental health services.
The hospital campus included several buildings, such as specialized units for treating tuberculosis and the criminally insane, which opened in 1924. The hospital also had a farm, known as Cherry Farm, spanning 2,300 acres, where patients worked as a form of horticultural therapy. The farm supplied the hospital with produce and livestock, contributing to its self-sufficiency.
In 1964, with the passing of the Civil Rights Act, Cherry Hospital was forced to desegregate and began serving patients of all races from 33 counties in the Eastern Region. Black patients were transferred to other hospitals, while Cherry Hospital received white patients. This marked a shift in the patient population and the hospital's history.
The hospital faced scrutiny in 2007 due to the case of Junius Wilson, who was falsely institutionalized for seven decades due to a communication disability.
